Admission criteria include:

• Primary diagnosis of intellectual, developmental and/or physical disability

• Age 18 or older 

• Medicaid-eligible or other approved funding source (regional funding or private pay)

• Meet criteria set by Iowa Department of Health and Human Services for intellectual disabilities

• Current physical examination by a primary care provider 

• Assessed needs that make one of our provided services appropriate 

• Referred by case manager or approved funder 

• Ability to indicate choices and assist in person-centered planning

• Ability to benefit from services 

• Other criteria as defined in application materials
